**Job Opportunity**
We are seeking a part-time Wellness Coordinator to cover a period of parental leave.
The role is due to commence in the near future and will be based at two retirement villages located in Prestons and Menangle.
The Wellness Coordinator will report to the Village Managers and provide effective assessment and planning to coordinate and support residents with individualised services to improve their independence, wellbeing and achieve their goals in life.
This includes identifying individual support needs, developing support plans, creating an environment that fosters resident participation and well-being, maintaining regular information/feedback to Village Managers, participating in leisure and recreational activities and establishing/maintaining a network with community and local service organisations.
**About the Role**
- Identify individual support needs, including goals, strengths and vulnerabilities of residents
- Develop support plans to coordinate access to services for residents
- Create an environment that fosters resident participation and well-being
- Maintain and provide regular information/feedback to Village Manager of potential changes to resident's situation
- Participate in leisure and recreational activities in relation to community inclusion as required by the Village Manager
**Key Responsibilities**
Experience in Retirement Living, Residential Care, Community Services, Home Care or a related discipline
Demonstrated ability to facilitate the provision of effective and individualised support aimed at achieving positive outcomes for residents
High level negotiation skills and ability to liaise with key stakeholders
Excellent communication skills - verbal and written, active listening skills as well as interpersonal skills
Tenacity, tolerance, empathy and patience with seniors
Valid Driver's Licence and access to a reliable vehicle
