Expert Palliative Care Specialist

Job Summary:
In this pivotal role, you will leverage high-level clinical expertise to support complex and evolving care needs. Your professional background in palliative care will enable you to thrive in residential aged care, hospital-in-the-home settings, and community clinics.
Key Responsibilities:- Undertake expert clinical assessments and develop advanced care plans
- Lead initiatives in evidence-based practice and service planning
- Contribute significantly to quality improvement and mentorship of staff
- Collaborate with the Community Palliative Care Team to enhance patient outcomes across care environments
- Registration as a Registered Nurse with the Nursing & Midwifery Board of Australia including endorsement as a Nurse Practitioner or working towards same as per NMBA Guidelines
- Highly developed skills in diagnostics, assessment, referral, and treatment planning
- A strong background in palliative care models and interdisciplinary teamwork
- Leadership capabilities with a focus on education, mentorship, and service development
- A valid NSW Class C Driver's License
