About the Role

We are seeking a highly skilled and compassionate Care Manager to join our team at Ozcare. As a Care Manager, you will play a critical role in planning, managing, and leading client care and support across a range of programs.

Key Responsibilities
  • Client Care Coordination: Manage and coordinate care services to ensure the highest standards of care for our clients.
  • Care Planning: Develop and implement care plans tailored to individual client needs, taking into account their unique circumstances and goals.
  • Communication and Collaboration: Liaise with clients, families, healthcare professionals, and other stakeholders to ensure comprehensive care and support.
  • Relationship Building: Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals.
  • Compliance and Quality: Ensure compliance with all relevant legislation and organizational policies, while continuously monitoring and evaluating care services to improve quality and efficiency.
  • Financial Management: Monitor the budget of Home Care Packages and ensure clients understand their funding and fees.

Requirements
  • Experience: Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
  • Communication Skills: Possess strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people.
  • Qualifications and Certifications: Hold current Police Screening Certificate, First Aid and CPR Certificate, NDIS Worker Screening Check (Yellow Card), Blue Card (Working with Children Check), and Queensland driver's licence.
  • Education and Experience: Possess tertiary qualifications and/or relevant experience in a health-related field, such as Health and Human Services.
