We are seeking an experienced and enthusiastic Assistant Care Manager to join our team at My Home Care Group. As an Assistant Care Manager, you will play a vital role in supporting our Care Management team and be mentored to take on your own caseload of aged care clients.

Key Responsibilities
  • Client Care and Support: Deliver exceptional client care for elderly Australians, ensuring their needs are met and exceeded.
  • Mentorship and Coaching: Be coached by our experienced Care Managers, receiving guidance and support to develop your skills and expertise.
  • Leadership and Management: Take on a leadership role within the myHomecare Group, overseeing the delivery of high-quality care services.
  • Care Planning and Coordination: Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
  • Collaboration and Communication: Work closely with our Nurses, Schedulers, Administrators, and Regional and other Managers to ensure seamless service delivery.
  • Financial Management: Monitor the budget of care packages and ensure clients understand service fees.
  • Compliance and Quality: Ensure compliance with all relevant legislative and industry standards, maintaining the highest level of quality in our services.
  • Continuous Improvement: Proactively engage in continuous improvement across practice and service delivery, driving excellence in our care services.
Requirements
  • Qualifications: Certificate III in Individual Support or similar qualifications.
  • Experience: Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired.
  • Skills: Frontline management skills, current First Aid and CPR certificates, and the right to work in Australia.
  • Personal Qualities: A current driver license, vehicle registration, and access to your own car with comprehensive car insurance, as well as a National Police Clearance or willingness to obtain.
