Position Overview: We are seeking a dedicated and skilled Care Management Assistant to join our Fully Managed Division. In this role, you will provide essential support to our Care Management team while being guided to manage your own portfolio of elderly care clients.

Key Responsibilities:

  • Provide exceptional care services to elderly clients.
  • Receive mentorship from experienced Care Managers to enhance your skills.
  • Act as a leader within the Fully Managed Division.
  • Oversee and ensure high-quality care delivered by our team and providers.
  • Utilize your customer service and administrative expertise to positively impact the community.
  • Develop, monitor, and adjust care plans in collaboration with clients, families, and healthcare professionals.
  • Work closely with Nurses, Schedulers, Administrators, and other Managers.
  • Manage the budget for care packages and clarify service fees to clients.
  • Ensure compliance with relevant regulations and industry standards.
  • Engage in continuous improvement initiatives to enhance service delivery.

Qualifications:

  • Certificate III in Individual Support or equivalent qualifications.
  • Experience in aged care, with familiarity in Home Care Packages and Aged Care Quality Standards preferred.
  • Strong frontline management capabilities.
  • Current First Aid and CPR certifications.
  • Eligibility to work in Australia.
  • A valid driver’s license and access to a vehicle with comprehensive insurance.
  • National Police Clearance or willingness to obtain.
