About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at Alzheimer's Queensland. As a Community Support Specialist, you will play a vital role in supporting individuals with dementia and their families to live fulfilling and engaged lives.

Key Responsibilities:

  • Provide person-centered support to clients in their homes, promoting independence and social engagement.
  • Assist with daily living tasks, such as personal care, domestic activities, and medication management.
  • Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals.
  • Provide emotional support and companionship to clients, promoting a sense of belonging and connection.
  • Collaborate with the multidisciplinary team to ensure seamless care coordination and communication.
  • Conduct regular progress reports and maintain accurate records of client interactions.

Requirements:

  • Relevant Certificate III minimum qualification in Aged/Disability or Community Care (or working towards).
  • Current National Police Check.
  • First Aid & CPR Certificate (willingness to obtain).
  • Drivers Licence & own Car.
  • Comprehensive Insurance.
  • Smart Phone.
