We are seeking a highly skilled Senior Clinician to join our team at Aged Care Resumes. As a Senior Clinician, you will be responsible for providing support to patients with complex health conditions, including care planning, palliative care needs assessments, and referrals to relevant health and community support providers.
Key Responsibilities- Develop and implement care plans to meet the needs of patients with complex health conditions
- Conduct palliative care needs assessments and develop collaborative treatment care plans
- Refer patients to relevant internal and external health and community support providers
- Provide education and support to other clinicians on complex patient needs
