About the RoleThe Discharge Coordinator - Associate Nurse Unit Manager provides professional high-quality patient care aimed at improved health outcomes. The successful candidate will assess individual patient needs, plan, and implement or coordinate appropriate service delivery from a range of accepted options.
You will have demonstrated competence in nursing practice in accordance with relevant standards, effective communication skills, conflict resolution, and negotiation skills that enable you to work effectively within a multidisciplinary team.
Key Responsibilities:- Deliver direct nursing care and/or case manage patients
- Assess patient needs, plan, and implement or coordinate appropriate service delivery
- Develop and maintain effective relationships with patients, families, and healthcare professionals
- Hold current registration as a Nurse with the Nursing and Midwifery Board of Australia
- Demonstrated experience in nursing practice, including competency in areas such as assessment, planning, implementation, and evaluation of patient care
