Patient Care Coordinator - Leura Wellness

Leura Wellness is an award winning boutique integrative health centre in Leura in the Blue Mountains, NSW. We integrate functional medicine, natural medicine as well as specialist rehabilitation medicine and IV nutrients.

**The Role**:
We are seeking a dedicated and compassionate Patient Care Coordinator to join our dynamic team. Your role will be pivotal in delivering outstanding patient experiences, coordinating care plans, managing patient interactions, and ensuring efficient communication between our healthcare providers and patients.

**Key Responsibilities**:

- Coordinate and manage patient appointments effectively.
- Act as the primary contact for patients, providing clear and empathetic communication.
- Collaborate closely with practitioners to coordinate individualised patient care plans.
- Maintain accurate and confidential patient documentation.
- Provide administrative support and assist with practice management.

**Who We're Looking For**:

- Previous experience in healthcare administration or a patient-focused coordination role essential.
- Experience with medical practice management software.
- Knowledge and passion for cannabinoid medicine
- highly preferred
- Strong interpersonal and exceptional communication skills.
- Excellent organizational abilities and meticulous attention to detail.
- Empathy and dedication to patient-focused care.
