Patient Care Coordinator

Duties and Responsibilities:
- Coordinate and schedule patient care staff to ensure timely delivery of dialysis treatments in contracted hospitals and facilities.
- Manage patient scheduling and staff utilization for maximum efficiency.
Key Responsibilities:
- Provide day-to-day guidance and support to outpatient care staff, ensuring they are equipped to deliver high-quality patient care.
- Collaborate with operations and clinical management teams to provide informal feedback and formal evaluations as needed.
- Assist in staffing decisions, including hiring, performance management, and disciplinary actions.
Requirements:
- Must be a graduate of an accredited School of Nursing (R.N.) with current state licensure.
- Minimum of 9 months nursing experience plus 3 months of specialized dialysis care in acute or chronic settings.
Work Environment:
- The work environment involves potential exposure to infectious diseases and materials.
- Employees must pass the Ishihara Color Blindness test.
