Senior Patient Care Coordinator

At HammondCare, we are seeking a dedicated and compassionate Care Manager to join our team.
This pivotal role involves delivering tailored care plans for each client on the program. The successful candidate will be responsible for ongoing assessment of clients' needs and preferences within Home Care.
The key responsibilities of this position include:
- Developing and implementing comprehensive care plans in consultation with key stakeholders
- Managing existing clients and their related Key Performance Indicators (KPIs)
- Ensuring that care plans are up-to-date and aligned with clients' needs, approval, and funding
In addition to these core duties, you will also provide professional and competent day-to-day delivery of services to clients in line with service expectations. This includes ensuring staff have a respectful understanding of individual clients' needs and preferences.
To excel in this role, you will need:
- A Certificate IV in Aged Care and at least 2 years of relevant industry experience
- Knowledge of the Home Care Packages Program, Commonwealth Home Support Programme, and Aged Care Quality Standards
- Experience working effectively in a multi-site environment
- A current driver's license and willingness to travel between clients, sites, and services
- Strong administrative and computer skills
