Empowering Lives with Disability Support

As a Disability Support Practitioner, your role focuses on empowering individuals with disability to lead fulfilling lives and take charge of their goals and aspirations.

This dynamic position requires delivering tailored support in a group home setting, fostering strong relationships with clients and helping them achieve independence through personalized care plans.

Key Responsibilities:
  • Providing individualized support to enable clients to engage with their community
  • Assisting clients with daily living tasks, medication administration, and manual handling using equipment and aids
  • Supporting clients to make informed decisions and develop skills for increased autonomy
  • Monitoring client well-being and reporting progress to stakeholders
Requirements:
  • Availability for a minimum of 3 days a week, with a total of 20-25 hours per week
  • A current NSW Driver's License (P2 minimum)
  • Experience working as a support worker in disability or nursing roles
