Job Description">

The Pastoral Care Coordinator provides support to clients, their families, team members and volunteers in palliative care service. They visit clients in their homes across the Northshore, promoting spiritual wellbeing and enhancing quality of life.

This role requires access to a car and involves working 22.8 hours per week, with one day on Wednesday.

Responsibilities

  • Provide pastoral care and support to clients, families, and staff.
  • Be a point of contact for internal and external support services.
  • Advocate for HammondCare's mission and values, promoting spiritual needs of clients.
  • Share expertise and coach staff to support client spiritual needs.
  • Participate in multidisciplinary team meetings and contribute to care planning.
  • Facilitate carer support groups and network with religious leaders.
  • Develop resources and activities to enhance client and family wellbeing.
  • Liaise with managers and staff, training them on spiritual care and wellbeing.

Requirements

  • Commitment to HammondCare's Christian mission and values.
  • Strong understanding of palliative care, dementia, and grief and bereavement.
  • Personal and active Christian faith, church involvement, and relevant qualifications.
  • Demonstrated capacity to pastorally care for those in need.
