
Palliative Care Specialist

As a Clinical Nurse Specialist, you will be working in a dynamic team to provide high-quality care to community clients. You will coordinate and lead projects related to patient flow and care coordination, monitoring patient wait times and estimated discharge dates through electronic patient journey boards.
The successful candidate will have experience working in palliative care and hold relevant post-registration qualifications. They will demonstrate effective communication and interpersonal skills, as well as critical thinking and analysis in their daily practice.
- Coordinate and lead projects related to patient flow and care coordination
- Monitor patient wait times and estimated discharge dates
- Work in a dynamic team to provide high-quality care to community clients
- Current registration as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
- Relevant post-registration qualifications and at least three years' experience working in palliative care
- Demonstrated ability to work within the interdisciplinary team at a senior level
