Job Title

We are seeking a seasoned Manager to oversee the delivery of high-quality disability services. This role involves managing client relationships, care plan review, and stakeholder engagement to enhance the client experience.

">Key Responsibilities:
  • Client relationship management to ensure exceptional customer service
  • Care plan review and management
  • Leading support workers in effective methods for delivering outstanding results
  • Service level monitoring, including funding review and client satisfaction
  • Ensuring consumer support plans and assessments are completed within required timeframes
  • Conducting case conferences with internal and external stakeholders
Requirements:
  • Demonstrated experience in Community Services Coordination or Case Management
  • Demonstrated experience in managing SIL/Disability accommodations
  • Certificate IV in Community Services or relevant discipline (Preferred)
  • Demonstrated experience supervising, supporting, and training staff
  • Knowledge of OH&S legislation relevant to aged care and disability service sector
  • Ability to work effectively as part of a multidisciplinary care team
