We are seeking a Support Worker to join our team and provide person-centered disability support services to NDIS participants.

The ideal candidate will have experience in Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation, Community Nursing, and Allied Health Services.

Key Responsibilities:
  • Assist with personal care and day-to-day activities in the home and community
  • Provide positive behavior support to clients
  • Complete documentation through shift notes, incident reporting, and data collection
  • Contribute to team objectives of providing best outcomes for clients
  • Work with team leaders to implement and review plans around daily service provision
  • Maintain professional relationships with clients and adhere to NDIS and code of conduct
  • Be responsive, reliable, and accountable to clients and team members

Requirements:

  • Minimum Certificate III in Individual Support/Aged Care or Higher (or ability to acquire)
  • Mental Health Experience
  • Stoma Care Experience
  • Problem-Solving Skills
  • Challenging Behavior Experience Verbal/Written Communication and Relationship Building Skills
