Job Title: Empathetic Client Liaison

We are seeking a highly skilled and empathetic client liaison to join our team. As a key member of our home care agency, you will be responsible for building strong relationships with clients and their families, ensuring their needs are met in a timely and compassionate manner.

In this role, you will conduct thorough client assessments, develop personalized care plans, and coordinate services to meet the unique needs of each client. You will also handle information requests, feedback, and general inquiries with professionalism and courtesy.

To excel in this position, you will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders. You will also have a strong understanding of aged care and disability systems, as well as experience supporting staff.

The successful candidate will be a natural relationship builder, with a passion for delivering exceptional client experiences. They will be highly organized, with strong problem-solving and time-management skills, and a commitment to upholding our agency's values of empathy, respect, and compassion.

Key Responsibilities:
  • Conduct client assessments and develop personalized care plans
  • Coordinate services to meet client needs, including rostering care workers
  • Handle information requests, feedback, and general inquiries

Required Skills and Qualifications:

  • Highly developed verbal and written communication skills
  • Strong organizational, problem-solving, and time-management skills
  • Current or willing to obtain Australian National Police Check and Working with Children Check
  • Current or willing to obtain First Aid and CPR certificate
  • Full Driver's license and reliable vehicle
