Job Overview
Supporting Child and Maternal Health in a Vibrant Community
- Undertake care coordination for a small caseload to ensure continuity of care across the healthcare journey.
- Identify and access appropriate referral pathways based on individual needs and goals of care.
- Liaise with families, carers, health practitioners, and others involved in providing services to Aboriginal mothers and children aged 0-4 years.
- Contribute to improved cultural competence and develop enhanced pathways with community health and wellbeing services.

Key Requirements
- Demonstrated ability to liaise and engage in a culturally sensitive manner with the Aboriginal community and individuals.
- High-level interpersonal, written, and verbal communication skills, including analytical commentaries and consultancy services.
- Negotiation and consultation skills with proven ability to engage and develop effective partnerships.
