Job Role

We are seeking an experienced and dedicated Care Coordination Specialist to join our team in providing comprehensive support services to Aboriginal women and their children affected by family violence.

The role involves working closely with Aboriginal families and communities to enhance child, family, and community capacity. Our specialist will work in partnership with the Senior Case Manager to address the health, well-being, and safety needs of clients.

Main responsibilities include:

  • Client Support: Provide regular contact with women and children at locations where they feel most comfortable and safe.
  • Stakeholder Engagement: Engage with key stakeholders directly related to the case management needs of clients.
  • Risk Management: Support community members to identify their safety needs and assist them to include these in their personal support plans.

In addition to the above responsibilities, the employee will be expected to perform other duties outside those set in the position description as directed from time to time which are within the employee's skill, qualification, experience, and competence level to meet the organisation's operational needs.

Selection Criteria
  • Qualifications: Relevant tertiary qualifications or demonstrated relevant work experience.
  • Cultural Awareness: Understanding of the historical and contemporary issues that impact on Aboriginal and Torres Strait Islander people and their communities.
  • Case Management: An understanding of family violence case management including risk and needs assessments, Women's and Children's interventions, and trauma-informed practice.
