Housing Specialist Case Coordinator

We are seeking a highly skilled and compassionate Supportive Housing Case Manager to join our team in delivering services to individuals experiencing chronic homelessness.
About the Role
The successful candidate will work with participants in an assertive outreach capacity, providing case management and service coordination activities, as well as liaison with other agencies and community resources.
- Key Responsibilities:
- Delivering flexible support packages to participants to sustain tenancies or move into stable housing.
- Building strong relationships with participants who have experienced chronic homelessness.
- Supporting participants to both move and settle into their tenancies and continue to link them with their local communities.
Essential Qualifications and Skills
- An appropriate degree qualification relevant to community services.
- Experience in case management and navigating complex service systems.
- A passion for sustaining tenancies and ending homelessness in Melbourne.
- Ability to adapt to participants' needs effectively and include the voice of the participant while collaborating with different services.
