Job Description:

We are seeking a highly skilled and compassionate Care Coordinator to join our team. As a key member of our organization, you will play a vital role in delivering high-quality care services to our clients.

The successful candidate will have experience in providing support and care for clients with empathy and expertise, improving their personal health and wellbeing goals. You will also be responsible for coordinating supports and wellbeing services for a highly engaged and motivated client population.

Key responsibilities include:

  • Providing initial early intervention triage with clients, supporting both physical and mental health services
  • Undertaking relevant case coordination activity, including liaison with all stakeholders and provision of email updates
  • Creating appropriate documentation, such as treatment plans or other documents as required

Required Skills and Qualifications:

To be successful in this role, you will require:

  • An experienced nurse with full registration
  • Mental health support service experience is highly desirable
  • A caring and empathetic nature
  • Experience in care coordination or occupational rehabilitation
  • Well-developed written and verbal communication skills
  • Good IT and time management skills
  • Strong customer service focus
