Care Coordinator Role

We are seeking a highly skilled and compassionate Care Coordinator to join our clinical team.

Key Responsibilities:

  • Supervise the day-to-day delivery of care and provide clinical guidance and support to care and allied health staff.
  • Ensure the delivery of person-centred care that meets the individual clinical, social, and emotional needs of residents.
  • Provide leadership, coaching, mentoring, and supervision to registered nurses.
  • Actively involved in the delivery of care and services to residents.
  • Promote and coordinate clinical documentation including assessment, care planning, and evaluation.
  • Review and manage incidents when they occur.
  • Support residents, families, and teams in coordinating case conferencing.

Requirements:

  • Relevant experience in a senior nursing position.
  • In-depth understanding of AN-ACC and quality assurance requirements that operate in aged care.
  • Strong supervision, organisational, and communication skills.
  • Current registration with AHPRA is required.
