About the Role


The Family Caring 4 Family (FC4 F) program aims to re-unify family under safe and thriving conditions. This initiative works to identify kin placement pathways for children and young people residing in the child safety residential OOHC system. The program places a great emphasis on family reunification with biological parents, when safe to do so.


Coordinated and culturally safe support and interventions are provided to families. By offering a comprehensive mix of services tailored to family to community needs, we aim to build family capacity, strengthen community connections, and safeguard the wellbeing of children.


In this role, you will effectively engage clients through cold-call home-based visits. You will turn initial 'kin agreement' home visit into ongoing consent and interest in becoming a Kin Carer. You will also actively seek contact details and relationship connection with child and broader family.


You will facilitate FLDM meetings with broader family kin, gaining family consensus on a Placement Plan to reunite child with family care arrangements. You will utilise the ATSIFLDM framework in facilitating meetings imbedded in the FC4 F Digital Model of Care.

Duties

Required Skills and Qualifications

  • Bachelor's degree in a relevant social services field with at least 4 years case management experience in family support, child protection, youth support, housing or related field.
  • Diploma in relevant Community/Services field with at least 6 years in a case management role in family support, child protection, youth support, housing or related field.
  • At least 6 years case management experience in family support, child protection, youth housing or related field.
