Family Support Specialist

This role involves working with families to provide support and services that promote family well-being and reunification.

Job Description:

  • Engage with families through home visits, outreach, and other community-based activities.
  • Assess the needs of each family and develop a plan to address those needs.
  • Provide individualized support to family members to enhance their ability to care for themselves and their children.
  • Facilitate communication between family members and service providers to ensure seamless coordination of care.
  • Monitor progress and adjust plans as needed to ensure successful outcomes.

Requirements:

  • Bachelor's degree in social work or a related field.
  • At least 4 years of experience in case management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in response to changing circumstances.
