
Reconnecting Families Coordinator

The Family Reunification Specialist plays a pivotal role in reconnecting families and strengthening community ties. This position requires exceptional case management skills, cultural awareness, and the ability to navigate complex systems.
Key Responsibilities- Engage with clients and their extended family members to identify kinship pathways for children and young people in the child safety residential out-of-home care system.
- Facilitate Family Link Day Meetings with broader family kin to achieve consensus on a Placement Plan and reunite children with family care arrangements.
- Assess carers through initial screening, Blue Card submission, and PAC assessments.
- Advocate and support families during external assessments.
- Deliver case-managed services according to the program model of care, ensuring equitable access to services for the community.
This role is part of a unique and supportive culture where every team member is valued as an individual. The ideal candidate will have a Bachelor's degree in a relevant social services field, at least 4 years of case management experience, and a commitment to complete a Diploma while working.
They will also need to identify as Aboriginal and/or Torres Strait Islander or be non-indigenous with at least 4 years of work experience in an Aboriginal Community Controlled Organisation.
Additional requirements include a Working with Children's Card, National Police Certificate, Driver's Licence, Evidence of Work Rights, and flexibility to work after-hours and on weekends.
