  • Main Responsibilities:
  • Build strong relationships with customers and support them to achieve their goals at home.
  • Conduct comprehensive assessments, develop holistic care plans, and coordinate multidisciplinary services.
  • Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
  • Support customers through care transitions, including hospital discharge or changes in health needs, and follow up where necessary.
  • Ensure accurate records are maintained in enterprise systems, participate in audits, and contribute to quality improvement initiatives.
  • Identify opportunities for additional services and support to enhance customer outcomes.
Your Skills and Qualifications
  • Essential Requirements:
  • Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
  • Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
  • Excellent verbal and written communication skills & technical aptitude with Microsoft Suite and CRM systems.
  • Knowledge of funding streams, quality standards, audits, and experience supporting care transitions.
  • Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
  • Collaborative mindset, able to work effectively in a team-based environment, liaising with families, clinicians, and external providers.
