We are seeking a skilled Home Care Specialist to join our multidisciplinary team. This is an excellent opportunity to make a real difference in the lives of patients and their families.
This role involves delivering high-quality care in patients' homes, working closely with healthcare professionals to achieve optimal rehabilitation outcomes.
About the RoleThe successful candidate will work across various caseloads, providing expert social work assessments, interventions, and patient management. They will also be responsible for collaborating with other healthcare professionals to ensure seamless care delivery.
This is a temporary part-time position available in Frankston, Mornington Peninsula, and surrounding locations. We offer a dynamic and supportive work environment that values professional growth and development.
What You'll Be DoingAs a Home Care Specialist, you will:
- Conduct comprehensive social work assessments to identify patients' needs and develop personalized care plans.
- Deliver evidence-based interventions to support patients' recovery, function, and quality of life.
- Collaborate with multidisciplinary teams to ensure continuity of care and optimize patient outcomes.
- Work with patients and their families to empower them to take control of their health and well-being.
You will also have the opportunity to work with patients on the Hospital in the Home client cohort, including those on the Home-Based Cancer Care pathway.
RequirementsTo be successful in this role, you will need:
- Highly developed communication and interpersonal skills.
- A strong understanding of subacute and community care settings.
- Experience working with patients with complex needs.
- A current Victorian driver's license.
