The role of a Clinical Care Coordinator is to provide expert clinical leadership to ensure high-quality care for residents. This position involves managing the needs of residents, documenting and assessing resident care, and collaborating with other teams to achieve best practice outcomes.
Key Responsibilities- Provide strategic clinical expertise to support the development and implementation of care strategies.
- Collaborate with the Care Manager to lead the carer team in delivering exceptional care services.
- Manage the assessment, documentation, and submission of care requirements within the facility.
- Support the effective documentation and implementation of policies, procedures, and IT system requirements.
- Coordinate care planning, review processes, and admission procedures to ensure optimal care assessments and initial consultations.
- Ensure accurate information is recorded and maintained in accordance with organisational and legislative requirements.
- Oversee the facility team rosters and ensure an optimal skill mix is available for delivery of care to residents.
- Identify and manage clinical risks within the facility, ensuring continuous improvement initiatives are implemented.
- Empathy and commitment to continually improving the quality of clinical care provided to residents.
- Minimum 1 year experience as a Care Coordinator or similar leadership position.
- Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
- Post graduate qualifications in aged care would be advantageous but not essential.
- Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
- Strong influencing and developing clinical leadership skills across the clinical care team.
- High level of computing skills to maintain documentation and reporting requirements.
- Confident team player with excellent communication and interpersonal skills.
- Ability to manage multiple stakeholders and handle emergency situations effectively.
- Valid NDIS Workers Screening Clearance.
