The Role

This full-time position reports directly to the General Manager, with support from the Clinical Care Manager. The key objective of this role is to ensure that all residents receive the highest quality of clinical care.

Key Responsibilities
  • Provide clinical expertise and leadership to healthcare teams.
  • Collaborate with the Care Manager to lead carer teams to meet all clinical needs of residents.
  • Manage assessment, documentation, and submission of AN-ACC requirements.
  • Ensure effective documentation and user support of policies, procedures, and IT system requirements.
  • Coordinate assessment, care planning, and review processes.
  • Support the admission of new residents for optimal care assessments and initial consultations with residents and families.
  • Ensure accurate information is recorded in accordance with organisational and legislative requirements.
  • Manage and document clinical risk within the facility.
Required Skills and Qualifications
  • A minimum of 1 year experience as a Care Coordinator or similar leadership position.
  • Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
  • Post graduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
  • An engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team.
  • A high level of computing skills to ensure documentation and reporting requirements are maintained.
  • A confident team player with excellent communication and interpersonal skills.
  • The ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency situations.
