Aftercare Services Lead

The Aftercare Services Lead plays a pivotal role in facilitating the delivery and coordination of aftercare services across the district. This position necessitates an experienced professional to collaborate with various stakeholders, including government agencies, health networks, and community service providers.

Key Responsibilities:
  • Coordinate stakeholder engagement and service integration to ensure consistent, high-quality care for individuals following a suicidal crisis.
  • Support referral pathways and service improvements through collaboration with multidisciplinary teams and community organisations.
  • Contribute to data reporting, evaluation, and quality initiatives to inform service design and implementation.
  • Promote awareness and understanding of aftercare services across the district.

This role will drive meaningful change and improve outcomes for people in our community by planning and implementing strategies to enhance service visibility, coordinating with district teams to ensure effective referral pathways, and playing a lead role in clinical and operational governance.
