Assertive Outreach Drug and Alcohol Clinician

The Assertive Outreach Drug and Alcohol Clinician provides evidence-based specialist healthcare, support, and care coordination to individuals living with complex unmet health and support needs who are experiencing primary homelessness in inner-city Sydney.
The successful applicant will assist clients to access mainstream and specialist healthcare and support services of their choice, to improve their health and wellbeing outcomes.
Key Responsibilities:
- Provide specialist assessment, treatment, referral, and care coordination services to individuals experiencing substance use and addiction, in addition to comorbid health and support needs.
- Work collaboratively with clients, multidisciplinary teams, and a range of stakeholders to address clients' cultural, holistic health, and support needs.
- Utilize principles of trauma-informed care and strengths-based, harm reduction best practice approaches to support clients.
- Develop and implement individualized care plans to improve clients' health and wellbeing outcomes.
- Collaborate with community-based services and organizations to provide comprehensive support to clients.
Requirements:
- Degree or diploma in a related health discipline with at least 3 years post-qualification experience and registration with a relevant professional body if applicable.
- Demonstrated competence, knowledge, and experience in providing community-based specialist assessment, treatment, and care coordination services to individuals experiencing substance use and addiction.
- Demonstrated understanding of and commitment to issues relating to homelessness, harm minimization, and trauma-informed care.
- Ability to work collaboratively and communicate effectively with clients, multidisciplinary teams, and a range of stakeholders.
- Demonstrated commitment to clinical supervision, ongoing professional development, and service improvement.
- Demonstrated effective computer literacy with email and an electronic patient record system.
