Healthcare Professional Opportunity

We are seeking a skilled and experienced Chronic Disease Management (CDM) Nurse to join our team at Murrumba Downs.

This role will see you support General Practitioners and their patients to identify and manage chronic conditions, ultimately keeping them out of hospital.

As a CDM Nurse, your key responsibilities will include:

  • Preparing Care Plans and Team Care Arrangements to help patients manage their disease/s
  • Empowering patients to take ownership of their health
  • Coaching patients on embedding health behaviour change principles
  • Enabling patients to access ongoing care from a multi-disciplinary team
  • Assisting in early identification of chronic disease by conducting health assessments
  • Working with General Practitioners to promote preventative healthcare throughout the practice
  • Increasing the number of patients who receive a care plan for the first time
  • Ensuring patient's care plans are reviewed regularly
  • Managing administrative tasks relating to carrying out CDM appointments

Successful candidates will have a proven track record in Chronic Disease Management and health assessments. They will also possess strong organisational and time management skills, as well as excellent communication and customer service skills.
