
Dedicated Chronic Disease Care Specialist

As a Chronic Disease Nurse Coordinator, you will play a vital role in improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key responsibilities include:
Clinical Leadership: Provide clinical leadership in care coordination, oversee comprehensive assessments, develop and monitor individual care plans, and support self-management through education and culturally appropriate health promotion.
Collaboration: Collaborate with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach. You will contribute to discharge planning and continuity of care, including liaising with community health providers and Aboriginal health services where relevant.

Essential qualifications and skills include:
Qualifications: The successful applicant must be eligible for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia.
Experience: A minimum of 5 years experience in senior nursing roles, preferably in a correctional setting. You will have demonstrated leadership and coordination of chronic disease program management and project management skills. Additionally, you will have developed strategies to prevent avoidable chronic conditions and possess initiative and problem-solving abilities.
