Healthcare Professional in Chronic Disease Management

We are seeking a passionate healthcare professional to work with clients who have been identified as having a chronic disease. The role involves working closely with multidisciplinary teams to develop and implement management plans and health checks, participation in disease-specific clinics, referrals to allied health services and regular review of recall registers.
Key Responsibilities
The successful candidate will be responsible for delivering quality care delivery and education surrounding chronic disease management. They will also participate in health promotion activities designed to promote community wellbeing through health information, promotional brochures, community development initiatives and self-care strategies.
Requirements
- Current APHRA registration as a Registered Nurse (Division 1)
- Experience working in chronic disease management, developing and managing chronic disease care plans and running effective chronic disease clinics
- Understanding of the health issues impacting Aboriginal and Torres Strait Islander communities
- Confidence using Microsoft Office and electronic client management systems
- The ability to work autonomously and take initiative, while contributing to a collaborative team
- A current Working With Children Check and Driver's Licence
- A willingness to obtain a satisfactory Police Check
