Disease Management Specialist

The organization is seeking a skilled and experienced Chronic Disease Nurse Coordinator to join the healthcare team within its Western Australian operations.
This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key responsibilities include clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans, and support for self-management through education and culturally appropriate health promotion.
Collaboration with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff will ensure a patient-centred approach.
Contribution to discharge planning and continuity of care includes liaison with community health providers and Aboriginal health services where relevant.
The role also contributes to policy review and development, including policies and procedures related to Chronic Disease Management.
