Clarence Community Health is seeking a dedicated Registered Nurse to join their Chronic Care Disease Management team. Our goal is to provide care coordination, case management, and support to individuals with chronic diseases, their caregivers, General Practitioners, and healthcare staff.
This position involves working closely with relevant stakeholders to ensure seamless service coordination, utilizing shared care plans and other collaborative approaches to effectively support people with chronic diseases within the framework of our Chronic Disease Management service.
About the Role
- Care Coordination: Provide comprehensive care coordination services to patients with chronic diseases, ensuring they receive timely and appropriate care.
- Case Management: Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
- Support Services: Offer emotional and practical support to patients and their caregivers, promoting independence and well-being.
