Job Description:

A Nurse Navigator is a critical member of the oncology team, working behind the scenes to provide exceptional patient care. This role involves coordinating care across the entire cancer treatment journey, identifying potential gaps in care delivery, and developing strategies to address these issues. The ideal candidate will have a strong background in oncology nursing, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.


Responsibilities:
  1. Perform coordination of care during the full continuum of cancer care experience.
  2. Identify potential and real gaps in care delivery; seek out opportunities for avoidance and/or closure.
  3. Prepare for, organize, and lead the Patient Care Conference (huddle) at regular intervals; communicate interventional outcomes to full care team in a timely manner.
  4. Perform assessment of patient risk stratification.
  5. Perform basic psychosocial assessments and provide emotional support; delegate and/or make referrals for additional screening and support as needed.
  6. Develop appropriate nursing diagnoses and corresponding action plans as needed.
  7. Support and/or provide oral chemotherapy education, adherence, and side effect monitoring.
  8. Hold or participate in survivorship and advance care planning visits; make referrals as needed.
  9. Participate in research, quality, and performance improvement, including reporting on navigation metrics.
  10. Support patient advocacy and empowerment.
  11. Participate in community outreach and cancer prevention education.

Requirements:
  • Bachelor of Science in Nursing or related degree is required.
  • Five (5) years of work experience as a Registered Nurse in an oncology practice or clinic.
  • OCN Certificate preferred.
  • Knowledge of environment and systems in which patients receive care.
  • Knowledgeable of practice operations guidelines, practice organization and health care economics.
  • Knowledge of resources to meet patient needs, taking into consideration health literacy, culture, language, and amount of information desired.
