About the Role:

">

The Healthcare Professional will be responsible for managing a cohort of Home Care Package Clients including:

">

- Care Planning: Manage client care plans ensuring all services and resources are utilized to enhance client health outcomes.
- Clinical Assessments: Conduct clinical assessments and provide hands-on care along with clinical guidance for other team members.
- Client Support: Serve as the primary contact for clients families and advocates.
- Onboarding: Lead the onboarding process for new clients
- Service Delivery: Oversee planning implementation and ongoing evaluation of care
- Ensure all client services are delivered in line with organisational policies compliance standards and budget requirements.

Requirements:

- Current AHPRA Registration Registered or Enrolled Nurse
- Experience in Case Management and Aged Care
- Highly developed communication and relationship management skills including strong documentation.
- Exceptional problem-solving skills and ability to relate with those navigating the Aged Care system.
- Demonstrated organisation and prioritisation skills.
- Agility and flexible in style
- Can do attitude.
- A professional attitude and presentation
