Expert Cancer Care Coordinator
Care for patients with cancer requires a unique blend of medical knowledge, emotional support and coordination skills. As a skilled nurse navigator, you will work behind the scenes to ensure seamless care delivery from diagnosis through treatment and beyond.
At the heart of your role is the patient. You will provide personalized guidance and support to help them navigate the complex healthcare system, ensuring timely access to quality care.

Responsibilities:
- Coordinate care across the continuum of cancer care, ensuring continuity and consistency of care delivery.
- Identify potential gaps in care delivery and develop strategies to address them, promoting high-quality care and improved patient outcomes.
- Lead Patient Care Conferences (huddles) to communicate interventional outcomes and facilitate collaboration among the care team.
- Perform risk stratification assessments to identify patients at highest risk and develop targeted interventions.
- Provide basic psychosocial assessments and offer emotional support, referring patients to additional resources as needed.
- Develop nursing diagnoses and corresponding action plans to promote optimal patient care.
- Support oral chemotherapy education, adherence and side effect monitoring, and facilitate survivorship and advance care planning visits.
- Participate in research, quality and performance improvement initiatives, including navigation metrics reporting.
- Advocate for patient empowerment and community outreach, fostering partnerships to enhance cancer prevention and control efforts.
Qualifications:
- Bachelor of Science in Nursing or related degree required.
- Five years of experience as a Registered Nurse in an oncology practice or clinic.
- OCN Certificate preferred.
- Knowledge of healthcare systems, practice operations guidelines, and health literacy principles.
- Ability to communicate effectively with patients, families, and healthcare teams.
