About the Role

The Coordinator Data and Evaluation Integrated Care Programs will develop, implement, and evaluate service development and process redesign initiatives to improve the efficacy and quality of Integrated Care programs. This role will be responsible for data collection, analysis, and the development of data management reporting structures and processes to enable effective monitoring and evaluation of Integrated Care service provision.

This position will work collaboratively with internal and external stakeholders to develop and deliver innovative projects and models of care across the organization. The successful candidate will have a proven ability to identify evidence-based practice gaps in service delivery and devise strategies to address these.

Key Responsibilities:

  • Develop, implement, and evaluate service development and process redesign initiatives to improve Integrated Care programs.
  • Collect, analyze, and present data to inform decision-making and drive improvement initiatives.
  • Collaborate with internal and external stakeholders to develop and deliver innovative projects and models of care.
  • Identify evidence-based practice gaps in service delivery and devise strategies to address these.
  • Prioritize, manage expectations, and make recommendations to optimize program outcomes.

Requirements:

  • Demonstrated tertiary qualifications and/or experience in data analysis and statistics.
  • Advanced skills in Excel, Power BI, data management, analysis, and data presentation.
