Homeless Outreach Worker Role

We are seeking a skilled and compassionate Homeless Outreach Worker to join our team in the Sunshine Coast region.

As a Homeless Outreach Worker, you will provide assertive outreach support to individuals and families experiencing homelessness or those at risk of homelessness within a holistic and collaborative multi-agency model of service.

  • Provide place-based assertive outreach to individuals and families experiencing homelessness or those at risk of homelessness
  • Support and refer clients to access and engage with services to address their individual needs
  • Work with other providers within a multi-agency team, to deliver a holistic and collaborative model of service
  • Provide emotional, social, and welfare support in a culturally safe, therapeutic, and trauma-informed manner
  • Timely and accurate completion of administrative tasks, including maintenance of service user records, meeting minutes, completion of daily logs, and required reporting
Requirements for Success
  • Demonstrated professional experience delivering case management to individuals experiencing homelessness
  • Strong interpersonal and written communication skills, and the ability to build rapport quickly
  • Strong stakeholder engagement skills within a multi-disciplinary setting
  • A Certificate IV or higher qualification in a relevant area of study is highly regarded
  • Experience working with people exhibiting challenging behaviors and complex needs
  • The ability to work independently to achieve client and program outcomes
Must Hold or be Willing to Obtain
  • Valid Queensland Driver's License with no restrictions
