**WHO WE'RE LOOKING FOR**

A outreach worker who loves to provide assertive outreach support to families experiencing homelessness or at risk of homelessness.

**WHAT YOU'LL BE DOING**

The position provides intake, assessment, casework and support services including referral under the Qld Government's Responding to Homelessness program which addresses homelessness and imminent risk of homelessness. Duties include the provision of information, advice, referral, advocacy and short term case planning and management.

**WHO YOU'LL BE WORKING WITH**

Reporting into the Outreach Team Leader of HART 4000, you will work alongside and in collaboration with our dynamic HART 4000 Homelessness Intervention Team.
