Age Care Professional Wanted

We are seeking an experienced Age Care professional to join our dedicated team. As a key member of our Aged Care team, you will be responsible for developing and implementing individual care plans that cater to each client's unique needs and preferences.

Key Responsibilities:

  • Conduct thorough assessments and create personalized care plans
  • Evaluate services against the Aged Care Quality Standards and ensure compliance with client rights
  • Coordinate funding and support services for participants
  • Lead and support a team of Support Workers in delivering high-quality care
  • Promote independence, safety, and well-being among clients

About the Role:

Requirements:

  • Degree in Nursing, Social Work, or a related field (or equivalent experience)
  • A minimum of 3 years' experience in aged care or community care coordination
  • Current driver's license and insured vehicle
  • Valid First Aid & CPR Certificate
  • Cleared National Criminal History Check and Working With Children Check
