Job Description

We are seeking a Care Coordination Specialist to join our team in East Perth Metro. The successful candidate will play a key role in helping families receive the care and support services they need in a timely and appropriate manner.

The Care Coordination Specialist will be responsible for developing and managing customer service schedules, Community Support Professional rosters and Coordinator day-to-day entering of schedule requests. This includes preparing and transmitting schedules for customer services to Community Support Professionals, arranging replacement staff to cover planned or unplanned absences and shift changes, liaising with customers and employees to ensure roster needs are met.

In addition, the Care Coordination Specialist will maintain and update data in company information systems, ensuring the specified standards for the delivery of quality in-home care and support services are met.

Required Skills and Qualifications
  • Caring and compassionate individual with strong interpersonal skills
  • Demonstrated high level verbal and written communication skills, including familiarity with Microsoft Suite
  • Proven relationship management with all internal and external stakeholders
  • Ability to work as part of a team and manage competing priorities
  • Experience with Procura scheduling system and current Police Clearance
