Our organization provides vital support to individuals in the mental health, disability, corrections and child safety sectors.

We specialize in assisting people with a broad range of physical, intellectual and psychosocial impairments through programs like the NDIS.

For those living with more complex impairments, we offer additional assistance to ensure they are accessing their NDIS funding effectively - this is where our Support Coordinators come in.

A Support Coordinator works directly with an individual receiving NDIS funding, helping them navigate their plan, connecting them to various services and relationships, and checking in to ensure their goals and aspirations are being met.

Currently, we have an opportunity for a full-time, permanent Support Coordinator in our North Brisbane team.

This role requires the ability to support individuals with complex needs, navigate multiple systems, and provide high-quality case management services.

Key Responsibilities:

  • Assisting individuals, their families and support networks to understand their NDIS plan;
  • Completing regular communication/reports with individuals, their families and NDIA;
  • Identifying suitable community services to meet an individual's needs;
  • Supporting individuals to communicate their goals and aspirations to stakeholders and the NDIA;
  • Attending home and community visits;
  • Managing general case management of an individual's portfolio; and
  • Monitoring individual NDIS Plan budgets.
