The Opportunity:
Melbar is looking for an energetic and driven Case Manager to join our supportive and dedicated team in a full time capacity. Our Case Managers play a vital role in assessing, coordinating, and managing the care services for our Home Care Package clients. As a Case Manager, your role will be supported by our service coordination (rostering) team, the finance team to manage reimbursement and Intake team for new clients to enable you to focus on providing quality case and client management service.
Your main responsibilities includes delivering a holistic approach to clinical and nursing care, focusing on the needs of clients, providing comprehensive, safe and effective evidence-based care to achieve individual health outcomes, incident reporting as well as reviewing and updating current and new consumers care plans.
Experience and Skills:
Relevant qualifications in Care with Community experience e.g. Certificate IV in Aged Care and/or Home and Community Services, Care management or Nursing
Demonstrated experience in a community/ nursing role;
Coordinating assessments & care planning;
Strong problem-solving and leadership skills;
Experience in care management and the Aged Care Quality Standards
High level of organisational skills and details orientated;
Excellent verbal and written communication skills;
Bilingual skills are essential with Mandarin as a compulsory language;
Valid Australian driver's license with access to own vehicle;
Competent computer skills in Microsoft Office software and ability to learn business systems quickly;
Demonstrate familiarity with Home Care packages, having previous experience in a home care environment will be a bonus.
