Job Opportunity

Housing and wellbeing are essential for the health and prosperity of Aboriginal and Torres Strait Islander communities in greater Brisbane and Logan. As a housing and wellbeing case manager, you will be responsible for providing support and interventions based on a program model of care to improve the lives of our clients.

Key Responsibilities:
  • To deliver high-quality interactions with clients that align with the values and vision of our organization.
  • To maintain accurate and up-to-date case management practices and records in line with operational requirements.
  • To facilitate client-led assessments and Care Plans that identify client goals, including whole family needs where appropriate.
  • To provide targeted interventions and family group meetings to achieve client goals.
Requirements and Qualifications:
  • A Certificate IV in Social Work, Human Services, or a related field, with at least 12 months of experience in a case management role.
  • OR a current graduate university bachelor's degree in social work, Human Services, or a related field, with at least a 10-week full-time placement in a social service field.

Ability to travel across the Logan and/or Brisbane region as per program location is required. A valid driver's license is also necessary.
