Mswa is seeking a highly skilled and compassionate Neurological Liaison Nurse to join our community nursing team. As a key member of our team, you will play a vital role in providing exceptional care and support to individuals living with neurological conditions.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of our clients.
- Provide direct nursing services and clinical care, including continence management, wound care, palliative care, and catheter care.
- Facilitate referrals to other necessary health services and collaborate with MSWA's Allied Health teams to ensure the highest level of care for our clients.
- Travel to clients' homes within the community to provide care and support.
- AHPRA Registered
- Current First Aid and CPR Certificate
- NDIS Worker Screening Check (or willingness to obtain)
- Full C Class driver's licence with no restrictions
- Working with Children Check
- Community nursing experience
- Background in neurological conditions or rehabilitation nursing
- Knowledge of Disability Services Legislation and NDIS
- Experience in palliative care, continence, and wound management
- Strong problem-solving skills and computer literacy
