Discharge Planner Specialist

We are seeking an experienced professional to fill the role of a Discharge Planner in our Social Work department.
Key Responsibilities:- Develop and implement comprehensive discharge plans for patients to ensure a smooth transition from hospital to community with appropriate care and services.
- Provide clinical leadership to nursing, medical, and allied health providers to coordinate patient discharge planning processes.
- Collaborate with stakeholders to identify and address barriers to patient discharge.
- Develop and maintain relationships with community service providers to ensure access to necessary resources for patients.
- Master's degree in Social Work or other relevant Allied Health qualification.
- Eligibility for membership of the Australian Association of Social Workers (or equivalent Professional Association).
- Extensive knowledge of community services and resources.
- Prior experience in discharge planning role.
- Excellent interpersonal and communication skills.
