About the Role

We are seeking a motivated and passionate healthcare professional to join our patient flow team as a clinical specialist focused on discharge/transfer of care.

The ideal candidate will work with patients to develop clear discharge or transfer of care plans, collaborate with a multidisciplinary team, and ensure timely referrals to other facilities.

Key Responsibilities:

  • Registration as a Healthcare Professional (level 1) with the relevant regulatory agency
  • At least 3-4 years of relevant clinical experience in healthcare delivery

Desirable Qualifications:

  • Post Graduate qualification in a relevant specialty or an equivalent level of experience in healthcare management
  • Experience working in the private healthcare sector
